Nursing Home Transition Diversion Waiver {NHTD}
Education and/or Experience
The individual who holds this position will be employed as the Assistant RRDS for the Nursing Home Transition Waiver and must:
- Have a high school diploma and a minimum of two years employment experience with people with disabilities and/or seniors in a community setting.
- Experience with consumer directed programs.
- Excellent understanding of community resources, human services and non-profit organizations.
- Be able to speak, read and write Spanish fluently.
Given the scope of responsibilities, to be qualified for the position of ARRDS, the individual must be an experienced professional with demonstrated skills and a dedicated commitment to working to support individuals with disabilities and seniors who wish to live in community based settings.
Qualifications
To qualify for the appointment to the ARRDS position, the individual must be able to perform each duty specified by the NHTD Waiver, satisfactorily and must have the following knowledge, skills and/or abilities.
- Have the ability to make clinical assessments and identify service needs.
- Ability to develop collaborative relationships with service providers and government entities to assure a comprehensive coordinated approach to service provision.
- Demonstrated commitment to integrated community-based services for individuals, their family members and other natural supports.
- Excellent organizational and training skills
- Recognized skill in coordinating activities, evaluating data and establishing priorities
- Excellent communication skills
- Excellent problem-solving skills
- Ability to multi-task
- Excellent screening and interviewing skills
- Successful experience with developing relationships with institutional and/or community-based organizations
- Working knowledge of services and supports available through Medicaid, Medicare and/or other third party payers
Responsibilities
- Assist RRDS with the coordination of all components of the Nursing Home Transition and Diversion (NHTD) Waiver on Long Island in conjunction with the Department of Health.
- Make referrals to available resources in the community if an individual is determined not appropriate for the waiver.
- Track intake and information calls and referrals
- Determine waiver participant eligibility. Complete enrollment procedures according to program guidelines.
- Make referrals to the appropriate LDSS to determine Medicaid eligibility; if appropriate, offer applicant the opportunity to participate in the NHTD waiver.
- Work in coordination with the LDSS to appropriately use Medicaid State Plan and NHTD waiver services for eligible applicants.
- Provide prospective participants with a choice among available waiver Service Coordinators.
- Review and monitor NHTD waiver participant Service Plans (under the direct supervision of the lead RRDS) developed by the participant and the Service Coordinator.
- Refer medically complex cases to the Nurse Evaluator.
- Work closely with DOH to disseminate public information and education regarding the needs of people with disabilities and their family members and regional community-based resources.
- Develop strong working relationships with regionally based inpatient rehabilitation units and long-term care facilities to identify prospective participants and facilitate community re-entry from these institutions.
- Provide data to RRDS or DOH as part of the quarterly reports and at other times upon request.
- Train service providers and offer technical assistance on the needs of the waiver participants, development and implementation of comprehensive waiver Service Plans, and standard documentation and reporting requirements.
- Maintain participant applications, Service Plans, and reports in a manner consistent with State standards for electronic transfer and information sharing.
- Make recommendations, based on experience with waiver activities and ongoing administration of the program, to RRDS and/or DOH for improvements to NHTD waiver policies and procedures.
- Assumes other related activities as needed by RRDS, DOH or Executive Director.
This is a non-exempt position.
