Welcome to SILO

Education ~ Advocacy ~ Empowerment

SILO is a 501(c)(3) non-profit Civil Rights, disability advocacy organization that was founded in 1983 by June Roberts.  For the past 25 years SILO has been providing its consumers with a broad range of programs and services so they can take control of their lives and become active members of the communities in which they live.  At the same time, SILO actively seeks to make systemic changes to increase access in the community for individuals with disabilities. SILO is Suffolk County's Independent Living Center (ILC).

New York State Nursing Home Transition & Diversion Waiver (NHTD) Medicaid Waiver Program at SILO

SILO is the Regional Resource & Development Center for NHTD program in both Nassau and Suffolk Counties.

 

What is the NHTD Waiver?

The Nursing Home Transition & Diversion Medicaid

Waiver (NHTD) is a program created to offer New Yorkers with disabilities and senior citizens a way to receive services in the most appropriate, least restrictive setting. The NHTD program offers a wide range of special services that will help people divert an admission to a nursing facility or promote transition from a nursing facility into living independently in the community.
